How Health System Aligned ACOs Have Calculated Meaningful ROI of In-Home Care

A May 2022 article in the American Journal of Managed Care analyzed over 150 accountable care organizations (ACOs). The headline finding was simple “Accountable care organizations deliver a diverse array of home-based services, but many of the services are not reimbursed. ACOs may not expand these programs without strong evidence of cost savings.” The article found that the majority of ACOs had an interest in offering more in-home services but demonstrating ROI from those services was a challenge. Although Medicare pays for in-home visits, there are inherent inefficiencies in traditional health-at-home models. Indeed, the lifeblood of many programs that emerged during COVID-19 was the public health emergency (PHE) waivers and payment for telehealth in conjunction with in-home primary care services.

This session will describe how two leading ACOs, Innovation Care Partners and MultiCare Connected Care, have partnered to deliver a comprehensive system of care in the home that addresses acute patients who can safely avoid the emergency room, hospitalizes (waiver and non-waiver patients) at home, provides transitional visits after acute stays, and delivers in-home diagnostics anchored on a purpose-built technology platform.

  • Formulate where in-home care strategies might make sense and think through build, buy or partner scenarios.
  • Compare why two leadings ACOs and their sponsor health systems have deployed in-home care strategies and measure ROI of these in-home care programs.
