Mapping the Patient Journey for Better Outcomes and ROI

In this session, attendees will learn how to identify and resolve key friction points along the patient journey, from long wait times at emergency department arrival to communication gaps during inpatient care and inconsistent follow-ups post discharge. By examining proven strategies, including AI-driven tools, interdisciplinary collaboration and evidence-based frameworks like Core Clinical Partners CARES (communicate, assess, revise, engage and sustain) model, participants will explore how optimizing both emergency and hospital medicine workflows directly impact patient satisfaction, clinical outcomes and performance. Engaging real-world examples and measurable metrics will illustrate how healthcare organizations can foster a culture of collaboration, reduce inefficiencies and improve throughput ultimately boosting ROI while delivering exceptional, patient-centered care.

  • Identify key friction points along the patient journey, including emergency department wait times, provider handoffs, inpatient care coordination and discharge inefficiencies that negatively impact patient satisfaction, clinical outcomes and performance.
  • Explore practical interventions such as AI-driven triage, standardized emergency medicine to hospital medicine handoffs, staff communication training and frameworks to improve throughput, reduce readmissions and enhance patient trust.
  • Understand how strategic emergency and hospital medicine collaboration can lead to measurable improvements in operational efficiency, including lower left-without-being-seen (LWBS) rates, optimized length of stay (LOS) and increased revenue while maintaining high-quality, patient-centered care.
