Delaware Valley ACO is one of the largest Medicare and commercial ACOs in the nation, managing a complex population of more than 300,000 patients. DVACO comprises two separate multihospital health systems (Jefferson Health and Main Line Health), including several thousand physicians, representing employed and independent practice models, primary care and specialty care. The differing cultures, supported by multiple data and electronic health record systems and numerous governance structures used by this diverse network of providers, made it difficult to drive performance improvement with existing analytic tools. DVACO needs stakeholders to engage with analytics at all levels, from operational to strategic, to be successful in achieving its mission to “accelerate the successful transformation to high quality value-based care in the region.”

 

This webinar will feature DVACO as a case study, whereby a joint venture ACO of two major regional systems executed a comprehensive clinical strategy powered by a homegrown advanced analytics platform to change the way care is delivered in the Philadelphia region. Participants will leave with knowledge of the success factors in transforming a diverse community of healthcare providers into a unified, high-performing value-based care network.
